Growth Hormone Secretagogues in Research: CJC-1295, Ipamorelin & Tesamorelin

Three of the most frequently researched compounds in the growth hormone/IGF-1 axis space — CJC-1295, Ipamorelin, and Tesamorelin — are all growth hormone secretagogues, meaning they’re studied for their ability to stimulate the body’s own GH release rather than supplying GH directly. But they work through different mechanisms and have very different evidence bases. This guide lays out how they compare.

Two different mechanisms, one axis

CJC-1295 and Tesamorelin are both synthetic analogues of growth hormone-releasing hormone (GHRH) — they bind the GHRH receptor on pituitary somatotroph cells and are studied for their effect on the frequency and duration of GH pulses. Ipamorelin, by contrast, is a selective ghrelin-receptor agonist (a growth hormone-releasing peptide, or GHRP) — it binds a different receptor (GHSR-1a) and is studied for its effect on the amplitude of GH pulses, with a well-documented pharmacological feature: minimal measurable effect on cortisol, prolactin, or ACTH, unlike some earlier-generation GHRPs.

How the evidence bases differ

This is where the three compounds diverge significantly, and it’s worth understanding before comparing them as a category:

  • Tesamorelin has the most substantial human clinical evidence of the three, including regulatory approval history under other brand names for a specific indication (visceral fat reduction in HIV-associated lipodystrophy), plus published trial data on hepatic fat and cognitive function.   • CJC-1295 has solid published human pharmacokinetic/pharmacodynamic data from a landmark 2006 study, but we did not find an independent peer-reviewed disease-outcome trial beyond that GH/IGF-1 axis characterisation.
  • Ipamorelin has its foundational receptor-selectivity pharmacology well established since 1998, plus a Phase 2 human trial specifically in postoperative gastrointestinal motility — a different application area from the fat/metabolic research associated with tesamorelin.

Why these are often studied together — and why that’s not the same as being tested together

CJC-1295 and Ipamorelin are frequently discussed as a pair because their mechanisms (pulse frequency vs. pulse amplitude) are considered pharmacologically complementary. It’s an intuitive rationale — but researchers should be clear-eyed that most of the literature studying either compound was generated with that compound alone, not in combination. A controlled trial specifically testing the CJC-1295 + Ipamorelin combination against either compound individually is not something we found in the published literature. Tesamorelin, meanwhile, has been studied exclusively as a standalone compound in every clinical trial we reviewed.

Which of the three has the strongest evidence base?
Tesamorelin, by a clear margin — it’s the only one of the three with a genuine regulatory approval history and multiple independent human clinical trials across different research questions (visceral fat, liver fat, cognition).

Are CJC-1295 and Ipamorelin interchangeable?
No — they act on different receptors (GHRH receptor vs. GHSR-1a) and are studied for different aspects of the GH pulse. They’re complementary in mechanism, not interchangeable.
