Mitochondrial research has become one of the more active corners of peptide science, and two compounds come up repeatedly: SS-31 (elamipretide) and MOTS-C. Both are studied in the context of mitochondrial function, but they’re structurally and mechanistically distinct, with very different evidence bases. Here’s how they compare.
Different origins, different jobs
SS-31 is a synthetic, lab-designed tetrapeptide engineered specifically to target the inner mitochondrial membrane by binding cardiolipin โ a phospholipid unique to that membrane. Its research focus is structural: stabilising mitochondrial membrane architecture and reducing oxidative stress. MOTS-C, by contrast, is a naturally occurring mitochondrial-derived peptide (MDP) โ encoded within the mitochondria’s own DNA โ that acts as a signalling molecule, travelling out to influence cellular metabolism and even entering the nucleus to regulate gene expression under stress. One is a membrane stabiliser; the other is a messenger.
Where the research has focused
SS-31 has the more extensive human clinical trial history of the two โ see our SS-31 Research Guide for the detail โ spanning primary mitochondrial myopathy, Barth syndrome, heart failure, and dry age-related macular degeneration. Importantly, that trial record is genuinely mixed: several major trials did not meet their primary endpoints, alongside others that reported positive results. MOTS-C’s research base is different in character โ a strong preclinical and mechanistic literature (its original 2015 discovery paper, exercise-physiology studies in aged mice) but, at time of writing, only one ongoing human interventional trial with no reported results yet. Why researchers shouldn’t treat them as interchangeable
Because both compounds get grouped under the broad “mitochondrial peptide” label, it’s tempting to treat them as variations on the same theme. The published literature doesn’t support that. They act through different mechanisms, are studied in different disease and physiology contexts, and โ critically โ have very different levels of human evidence behind them. Researchers designing a study or interpreting existing data should treat each compound on the strength of its own published record, not by association with the other.

Is one of these compounds “better researched” than the other?
SS-31 has more completed human clinical trials, but with mixed (not uniformly positive) results. MOTS-C has a strong preclinical literature but minimal completed human trial data. Neither should be considered to have an established, positive human efficacy record at this stage.
Are SS-31 and MOTS-C ever studied together?
We did not find any published controlled study combining the two. All cited clinical and preclinical work studies each compound as a standalone agent.
